What is needed for a Medallion Signature Guarantee?

This service is available by appointment only to existing Verve members whose accounts have been open for a minimum of 30 days and are in good standing.

The documents required are as follows:

  1. Form(s) requiring the Signature Guarantee Stamp
  2. Proof of Authorization to Transact (if needed)
  3. Proof of the value of the transaction
    1. Quarterly Statement
    2. Letterhead document with all pertinent information
